At Elements, our care extends not only to the patient but to the loved ones on the journey as well. We are committed to providing education, ease, and empathy during the aging process. We understand the importance of eldercare and know the kind of care we would expect for our own loved ones. PRIVATE DUTY CARE

Private duty assistance is typically provided to elderly individuals who need help with day-to-day activities. Housekeeping, meal preparation, medication reminders, transportation, and personal care such as bathing, dressing and hygiene are all tasks that your private duty attendant can assist you with!

TRANSITIONAL CARE

Transitional care includes skilled nurses assisting you after a fall, or a speech therapist who teaches relearning of language after a stroke. Transitional healthcare includes everything from physical therapy, wound care, and any care regarding helping the patient return to a normal lifestyle.

HOSPICE CARE

“I’ve lived a good life – but my disease is taking over who I am”

Let us help change that!

Patients and their families call on hospice when curative treatments are no longer desired. Quality of days instead of quantity of days have become the new healthcare goals at the hospice stage. Hospice staff provide care at the final stages.
