Transitional care involves the change of setting to a patient after leaving a medical hospital after a stroke, slip and fall, or other illness. As people get older, their health changes, therefore their healthcare and support needs also changes. Here at Elements of Elder Care, we provide care to help our patients as they transition from the hospital back to their home. We provide continued care so they feel safe and cared for during an already stressful time. Transitional care is designed to support the specific healthcare plan that is already in place for an individual. It is time-limited care that is provided as a filler in between stages of healthcare. The intent is to bridge the gap between levels of care. WHAT KIND OF TRANSITIONS ARE THERE?

Transitional care can look different in a variety of ways depending on the needs of each individual patient. Transitions in and between settings are very common during someone’s healthcare journey. Some examples of changes in setting include home to assisted living, primary care to specialty care, senior center to hospice. Many other settings are included when it comes to change of care based on one’s setting. Transitional care also includes changes in the level of healthcare provided. Examples of changes in healthcare include more intense care in private duty care, general practitioner to specialized practitioner, in or out of physical therapy, and even changes among nurses or physicians.

HOW LONG IS TRANSITIONAL CARE TYPICALLY NEEDED?

Transitional care is designed to be time-limited and based on the needs of our patient’s and their healthcare plan. It could be as short as a few weeks to months. If they are leaving the hospital and can go home, we come to you, to support your loved ones with doctor’s appointments and medications. If they are in an assisted living facility, we can also provide that extra care that they need there as well. We can take them to physical therapy, occupational therapy, and other healthcare appointments. As the needs of our patients change, we can help guide you all through all the elements of your life.
