WHAT IS IN-HOME HOSPICE CARE?

In-home hospice care is where our team of healthcare professionals serve you or your loved ones wherever you reside in order to reduce pain and provide for their physical and mental needs. We will come to you wherever your home is, whether that be in home, assisted living, a group home, etc. Our staff can also help provide for their psychological and spiritual needs as well. Hospice care is not focused on a cure for illnesses, but rather providing comfort and support for the remaining time a patient may have. Like other forms of care, hospice also involves a team of caregivers. This team can include, but is not limited to nurses, doctors, counselors, pharmacists, spiritual counselors, social worker, and other health aides. Unlike palliative care, hospice care no longer attempts to cure our patient’s illness.

WHEN TO CHOOSE HOSPICE CARE?

Typically, hospice care is given when a patient has 6 months or less to live. Hospice care can also be given when a patient rapidly declines in their health and medications are not helping. Some illnesses reach a point where there is no longer a cure, or the patient does not want to accept certain treatments for their illness. Unlike palliative care, hospice care is offered when treatment of an illness is no longer happening. Instead, it treats symptoms rather than reaching a cure. Patients of terminal illnesses can reach a point where they feel like their illness is taking over who they are. At Elements of Elder Care, we want to make the last stage of one’s life as comfortable as possible and for as long as possible. Hospice care is designed to prepare our patients and their families for the end of their life.

It can also provide more time to be with family if started early enough. Families end up being more satisfied with their loved one’s end of life care when they enter hospice. Patients who receive hospice care also tend to have their pain and symptoms better managed under our care. Since they are receiving personalized care to better their quality of life.

WHAT DOES HOSPICE CARE COST?
